Nursing Services Requirements for the State of New Hampshire

He-P 803.03 Personnel and Staffing

(k) The nursing department shall be under the supervision of a director of nursing. The director of nursing shall be a registered nurse currently licensed in the state of New Hampshire pursuant to RSA 326-B and have at least 1 year of experience in long term care.

(l) There shall be at least 1 licensed nurse per shift every day and staffing adequate to attain or maintain the highest practicable physical, mental and psychosocial well-being of each resident.

(m) There shall be at least 1 registered nurse employed 7 days a week, 8 hours a day.

He-P 803.04 Resident Management

(o) The nursing staff shall provide nursing and related services to attain or maintain the highest practicable physical, mental, and psychosocial well-being of each resident, as determined by by resident assessments and individual plans of care.

(p) Nurses notes shall be recorded, signed and dated, placed in the residents clinical record and include when applicable:

(1) The general condition of the resident, including any significant condition changes

(2) Needs, unusual symptoms and behavior;

(3) Appetite, with routine monitoring of weights;

(4) Response to care;

(5) Progress towards meeting care plan objectives;

(6) Rehabilitative nursing care services performed and resident response to these services;

(7) Notation on resident's health related visits to clinics, hospitals or other health care facilities;

(8) Licensed practitioner notifications regarding changes in resident's condition;

(9) Notation of unusual incidents and accidents including:

a. actions taken;

b. notification of licensed practitioner;

c. notification of family;

d. notification of resident's representative, if appropriate.

(10) Effects of medications and treatments and licensed practitioner notification of undesirable side effects;

(11) Notification of the resident's representative if any of a significant change in resident's physical, mental or psychosocial status or a need to alter treatment significantly;

(12) Notification of the resident, or resident's representative, prior to transfer, of the reason for the resident's transfer or discharge.
