Summary of Content: Serve as research consultants to RWJF as part of the Regional Market Pilot. The reserachers will assess the Regional Market design for potential weaknesses and provide frequent evaluation insight to the program development team. At the same time, the evaluation team will be planning an evaluation of the future phases of this initiative. The Regional Market Pilot will promote the redesign of a community’s care and care system processes by activating and empowering health care consumers and providers using best practices, appropriate information technologies, coordination of care settings, performance and outcomes measurement, comparative price and performance information, and alignment of payment policies to improve care. RWJF intends to test the hypothesis that harnessing market forces in a given communityand aligning them with local quality improvement efforts has the best potential to produce sustainable high quality and efficiently delivered health care.

Project Name: Full-Scale Implementation and Sustainability of Small Group Nursing Homes in Tupelo, MS: Lessons for Replication

Summary of Content: General Description and Aims. Through interviews with key informants, observations, and secondary analysis we will describe the conversion of the entire Cedars Health Care nursing home in Tupelo, MS into small group nursing homes, emphasizing the key decisions made, the challenges encountered, and the generalizable lessons. Themes underlying the exploration include organizational structure, role differentiation, sustainability, market acceptability, and individuation of the homes. We have laid out 4 general tasks 1. Examine the overall functioning of the NH, including how standard functions and roles required of any nursing home have been managed for the 10 regular GHs and any sub-acute GHs developed in Tupelo? (This task seeks lessons related to the overall organization.)2. Examine the internal functioning of the GH to identify how the operating structure and philosophy of the house is developed and sustained, and how much differentiation and innovation is found in both the 10-elder and 12-elder GHs in staffing patterns, programs, and incorporation of new staff and residents intothe house? (This task seeks lessons related to internal GH functioning.)3. Examine case-mix adjusted quality indicators for the GH nursing home.4. Extrapolate the implications for the GH model in the views of those who developed it.
